The Triple A’s of Geriatric Care Management:
Assess • Advocate • Assist
At Litherland, Kennedy & Associates, APC, we understand the challenges that come with aging—not only for seniors but also for their loved ones. That’s why we offer professional Geriatric Care Management services through our experienced and compassionate Geriatric Care Manager, Dedra Jize.
Dedra works closely with seniors and their families to help them navigate the complexities of aging, health care, housing, and support services. Whether you are a client of our law firm or simply in need of expert guidance, Dedra is here to provide clarity, direction, and peace of mind.
Assess
Every individual has a unique set of needs, goals, and circumstances. Dedra begins by conducting a thorough assessment—whether the person is at home, in a hospital, or in a residential facility. Her goal is to understand the full picture: health status, safety, daily living abilities, cognitive and emotional well-being, and available support systems.
From there, she identifies what’s needed to help that individual remain safely at home—or determine if another living situation would better support their well-being. Her recommendations may include:
-
In-home care services
-
Medical equipment and home modifications
-
Rehabilitation and therapy
-
Social engagement opportunities
-
Senior center or adult day programs
-
Transitioning to the right type of senior living community
Dedra provides a personalized care plan that reflects not only medical and physical needs but also emotional and social well-being. Her goal is always to promote independence while enhancing quality of life.
Advocate
Navigating the healthcare system can be overwhelming. Dedra serves as a dedicated advocate, making sure your voice—and your wishes—are heard. She communicates with doctors, discharge planners, and case managers to ensure your care aligns with your goals.
For families living far away or unable to attend appointments, Dedra becomes a trusted liaison, relaying important updates and helping coordinate care. She works alongside you to develop and implement a care plan that evolves with your needs, always championing your preferences and priorities.
Assist
From identifying resources to facilitating access to care, Dedra is here to help every step of the way. She connects families with trusted service providers and helps evaluate financial options such as:
-
Long-term care insurance
-
Reverse mortgages
-
Veterans’ benefits
-
Medi-Cal and other public programs
Dedra is also experienced in assisting with Medi-Cal applications and, when needed, can represent clients in Medi-Cal meetings—so you never have to face the system alone. When legal guidance is required, she works hand-in-hand with our attorneys to help you make informed decisions that protect your loved one’s assets and well-being.
A Trusted Partner Through Life’s Transitions
Aging can bring uncertainty, especially when a loved one needs care and doesn’t recognize it—or when family members are unsure where to turn. Dedra brings knowledge, compassion, and calm to even the most difficult situations. She supports both seniors and their families by offering thoughtful solutions and steady guidance through life’s transitions.
At Litherland, Kennedy & Associates, we are committed to providing holistic care—legal, practical, and emotional—so our clients and their families can move forward with confidence.
You don’t have to navigate this journey alone. Dedra is here to Assess, Advocate, and Assist—every step of the way.
